Rental Request Form
    • I assume responsibility for my actions and for any injury, loss, or damage arising from my use of the Church facilities. I agree to release and hold harmless St. Peter's Lutheran Church from liability to the fullest extent permitted by law.
    • Unless Clean Up is included in the rental, I agree to remove all trash and return all tables, chairs, and equipment to their original locations and condition before leaving and that any such work is expected to be conducted with the rental time.
    • I understand the maximum rental period includes the time needed for my group to setup, cleanup, and returning the room to its original condition.
    • I understand that a payment equal to 50% of the total rental fee is required to reserve the facility.
    • I understand that a $50 cancellation fee will be charged if the reservation is canceled within 15 days of the scheduled event.​
